Application Checklist

Please submit your completed application along with all required documents to the Office of Admissions. You are encouraged to contact H.M.I. directly to determine whether additional admission materials are required beyond those listed below. To ensure prompt processing of your application, please follow these instructions carefully.

Submit all of the following to:

  • Completed, signed application
  • Two Passport size photos (with your printed name and signature on the back)
  • $75.00 non-refundable application fee (made payable to Hope Medical Institute)
  • Two original or notarized copies of your birth certificate
    (Original birth certificates may be obtained from the Department of Health or Department of Vital Records in the state you were born in)
  • Two notarized copies of your passport page that shows date of birth
  • Essay – “Why I Want To Become A Physician”
  • Two official high school transcripts, sent directly from the institution
  • Two notarized copies of high school diploma
  • Two official transcripts from each university/college attended, sent directly from the institution
  • Two notarized copies of all degrees you have received
  • Official or notarized SAT or ACT scores (if the exam was taken)
  • Official or notarized MCAT, VCAT or PCAT scores (if the exam was taken)
  • Two original, signed academic letters of recommendation
All applicants can submit unofficial/student copies of transcripts for processing only.

However, if admission is granted, official copies of transcripts will be required and should be sent to Hope Medical Institute directly from the institution. Transcripts downloaded from the Internet will not be accepted. Letters of recommendation can be sent via e-mail by the author for processing only.

Please note that college graduates who have already received degrees must also provide a notarized copy of their high school diploma and transcripts or can have official copies sent directly from the institution.

In addition, please make sure that your application form is filled out completely and that you have attached a current passport size photo (with your printed name and signature on the back) with a paperclip. Please do not tape, staple or glue your photo to the application.
